GUIDELINES FOR REGISTRATION

 

  1. The child must accompany the parent/guardian at the time of registration.
  2. The following documents are mandated in order to register your child:
    • Proof of address (any of the following is appropriate)
      • A utility bill in the parent/guardian’s name (gas or electric).
      • Document from a federal, state or local agency i.e. IRS, HRA, etc.
      • A lease in the parent/guardian’s name.
      • A letter from the parent/guardian’s employer indicating the address you reside at.
      • If you do not have any of the aforementioned proof of address due to subletting an apartment, living with a relative or renting a room, YOU MUST present a Department of Education Parent Affidavit of Residency Form along with a utility bill in the landlord’s or primary leaseholder’s name.
      • Please Note:  The following are unacceptable proofs of address: telephone bill, cable bill, credit card bill or driver’s license.
    • Proof of birth (passport or birth certificate)
    • Complete and up-to-date immunization record (TDAP, Varicella and Meningitis are mandated vaccinations for all children turning 11 years of age).  A Physical Examination Form is mandatory for all children entering the NYC School System for the first time.
    • Latest Report Card from Previous School (this is to ensure proper class placement)
  3. Uniform Policy – All students are required to wear the school uniform which is:  Khaki pants, White or Burgundy Polo Shirt, Black Sneakers only (no colors), Burgundy Sweater or Sweatshirt.  Three of the local stores where you can purchase the uniform are Jackie’s Kids or Young World on Junction Boulevard or Old Navy on 82nd Street.
